Questions for your doctor or pharmacist

Mark the questions you want to ask. This is a conversation aid, not a prescription or a test of whether a medicine is safe.

Questions to discuss at a medicine appointment

Notes to write after printing

Educational checklist compiled by the PakVita Editorial Team, 13 October 2026. Adapted in original wording from FDA patient resources linked on this page. Follow advice from your own qualified clinician.

How to use the sheet

  1. Tick the questions you want to raise; the full sheet stays visible so you can also add questions after printing.
  2. Choose Print checklist or your browser print command. Use print preview and choose your paper size before saving as PDF or printing.
  3. Bring the sheet and your current medicine list or packs. Keep patient-specific information off this website and discuss it privately.
  4. Write down the professional's answers. If an instruction is unclear, ask them to explain it again rather than guessing.

Use it alongside a medicine profile

A profile can help you recognise an ingredient name or understand a term on a pack. It cannot account for your full history or select your treatment. Bring any mismatch between the pack, prescription and website to a pharmacist. The questions here help organise that discussion; ticking them does not certify that a medicine is safe for you.

For an urgent health problem, seek medical care instead of waiting for an email reply. This sheet is not a medicine schedule and does not calculate doses or check drug interactions.

Patient information behind the checklist

The prompts are written for this worksheet using these FDA patient-communication resources. They describe general conversation practices; the FDA is a US regulator and does not approve PakVita or establish Pakistani product registration.
