Theophylline Tablet Uses (Fayde / Kis kaam aati hai)
Theophylline (Theo-Dur, Phyllocontin) is a xanthine bronchodilator used in Pakistan. All its approved uses and important limitations are detailed below.
Primary Uses (Mukhya Istemal)
COPD (Adjunct Bronchodilator)
Theophylline is used in COPD patients when inhaler therapy (SAMA, LAMA, LABA, ICS/LABA) does not provide adequate control. It relaxes airway smooth muscle. Serum level monitoring is necessary.
Asthma Maintenance (Second/Third-Line)
Modern ICS/LABA inhalers (Symbicort, Seretide) have replaced theophylline in asthma. It is now a second or third-line option — mainly in resource-limited settings or when ICS/LABA is not tolerated.
Neonatal Apnoea (Hospital Only)
IV aminophylline is used in hospital for apnoea in premature neonates — strictly a specialist/neonatologist domain.
Who Should NOT Use (Kin ke liye nahi)
- Patients without serum level monitoring capability
- Severe uncontrolled seizure disorder
- Serious cardiac arrhythmias
- Patients who can be managed adequately with modern inhaler therapy
Modern vs Theophylline
| Condition | Preferred Option | Theophylline Role |
|---|---|---|
| Asthma (reliever) | Salbutamol (SABA) | Not recommended |
| Asthma (maintenance) | ICS/LABA | Second/third-line only |
| COPD | LAMA/LABA combinations | Adjunct if insufficient |
Summary
Theophylline is used as an adjunct bronchodilator in COPD and as second/third-line maintenance in asthma. Due to its narrow therapeutic index, serum level monitoring is necessary. Do not take without a doctor's guidance.
